What’s Next?
Renew my registration or my
card that’s about to expire?
Contact your practitioner to extend
the expiration date on your current
certification.
If your practitioner extends the
certification before it expires, you
will not need to re-register and you
will not need a new registry ID
card. The same is true for your
designated caregivers.
Renew my card that has
expired?
Obtain a new certification and
register on-line in MCDMS by
logging into your https://my.ny.gov/
account.
When your registration is
approved, a new card will be
mailed to you. You will have
immediate access to a temporary
ID card that is valid for 30 days.

Change my street address?

navigate to your registration and
request a new card.
You will need to upload supporting
documents for your new
residence. The Office will review
your new card request.

navigate to your registration and
request a new card.
The system will determine if a new
card is needed.
Change my name?
Log in to https://my.ny.gov/,
navigate to your registration and
request a new card.
You will need to upload supporting
documents for your new name.
The Office will review your new
card request.
Update the photo for my card?
Log in to https://my.ny.gov/,
navigate to your registration and
request a new card.
You will need to upload a new
photo or select to use a photo on
file with the NYS DMV. The Office
will review your new card request.
When approved, a new card will
be mailed to you. You will have

Print my Temporary Registry ID
Card?
Log in to https://my.ny.gov/ and
click on the Temp Card icon on
your registration tile.
If the Temp Card icon is not there,
it means that the temporary ID
card has expired (30 days).
